Chapter 1

The cardiac monitor screamed.

I pulled the sutures tight, fingers moving on instinct while my mind stayed three steps ahead. Trauma Bay 2 smelled like blood and antiseptic. The gunshot victim on my table had a lacerated ventricle. I had maybe four minutes before we lost him.

"Clamp," I said.

Claire Sato slapped it into my palm without a word. We'd worked together long enough that she knew my rhythm better than I did.

The ER doors exploded open behind me.

"Cardiac patient incoming!" someone shouted. "Twenty-eight-year-old female, syncope at the Regency premiere afterparty, presenting with severe arrhythmia—"

I didn't look up. My world was the chest cavity in front of me, the torn muscle I was rebuilding stitch by stitch. The chaos behind me was someone else's problem.

"—BP dropping, we need a cardiothoracic consult now—"

"Dr. Burke!" That was Victor Hale's voice, sharp with the particular panic of an administrator who'd just realized celebrities were involved. "We need you out here."

I tied off the last suture. Checked the repair. The bleeding had stopped.

"He's stable," I told the surgical resident across from me. "Close him up. I want hourly vitals for the next six hours."

I stripped off my gloves and pushed through the doors into the main ER.

The scene hit me in fragments. Gurney. Crash cart. A woman's face, pale and beautiful even in distress, dark hair spilling across the pillow. Expensive dress. Camera flashes still going off outside the glass doors because of course the paparazzi had followed the ambulance.

And standing beside the gurney, hands gripping the rail like it was the only thing keeping him upright—

My heart stopped.

Damian.

Eight years collapsed into nothing. I was twenty-three again, exhausted and hopeful, watching him sleep in our apartment with morning light on his face. I was twenty-four, alone in an ER at 3 AM with my hand wrapped in gauze, calling a number that went straight to voicemail.

I was thirty-one, and he was looking at me like I was a ghost he'd been running from his entire life.

His face had changed. Sharper now. Famous. I'd seen it on magazine covers at the grocery store, on billboards in Times Square. I'd gotten very good at looking away.

But his eyes were the same.

I held his gaze for exactly two seconds. Let him see that I recognized him. Let him see that it didn't matter.

Then I turned to the ER attending. "What do we have?"

The attending—a capable second-year named Rodriguez—looked relieved to focus on medicine instead of celebrity drama. "Lilith Vasquez, twenty-eight, collapsed during a red carpet appearance approximately forty minutes ago. Witnesses report she clutched her chest and lost consciousness. Paramedics found her in ventricular tachycardia, cardioverted once in the ambulance. She's stable now but the rhythm's still irregular."

I moved to the gurney. Checked the monitor. Lifted her wrist and felt her pulse—rapid, thready, but there.

Lilith Vasquez. I knew the name. Everyone did. Rising star. America's sweetheart. Dating Damian Reynolds for three years, if the tabloids were accurate.

Of course they were together.

Of course.

"Run a full cardiac panel," I said. My voice came out steady. Clinical. Perfect. "CBC, metabolic panel, troponin levels, and get me an EKG and an echo. I want tox screens for stimulants. Check her medication history—could be a reaction, could be congenital. We'll know more in an hour."

Rodriguez nodded and started calling orders.

I stepped back from the gurney. Damian was still standing there, frozen. His mouth opened.

I walked away.

Down the corridor. Past the nurses' station where Claire was already pulling up Lilith's chart. Past the residents' lounge. I didn't stop until I reached my office, closed the door, and locked it.

Then I stood in the dark and breathed.

My left thumb found the scar on my right hand. The ridge of tissue that ran from my wrist to my knuckle, pale and permanent. I traced it slowly. A habit I'd never broken.

Eight years.

I'd rebuilt myself from nothing. Finished my residency at the top of my program. Became one of the best cardiothoracic surgeons in New York. I didn't think about him. I didn't look him up. I didn't wonder.

And now he was standing in my ER, looking at me like I was the one thing in the world he couldn't fix.

I sat down at my desk. Opened my laptop. Stared at the screen without seeing it.

Somewhere down the hall, Lilith Vasquez was stable and resting. Damian was probably still in the waiting area, doing whatever famous people did when their girlfriends collapsed at premieres.

It wasn't my problem.

None of it was my problem.

I pulled up patient files and started reviewing tomorrow's surgeries. Work was the only thing that had never betrayed me. Work was the only thing I could trust.

My thumb traced the scar again.

I didn't even realize I was doing it.

---

The next morning, I arrived at the nurses' station at six. Early, even for me. I hadn't slept.

The hospital was quiet. Pre-dawn shift change. The overhead lights hummed.

I stood at the counter and stared at the schedule board without reading it.

Footsteps behind me.

Grayson Tucker appeared at my elbow and set a coffee cup beside my hand. Black. No sugar. The way I always drank it.

He didn't say anything. Didn't ask if I was okay. Didn't mention that the entire hospital was buzzing about Damian Reynolds and Dr. Burke's history.

He just stood there, drinking his own coffee, looking at the same schedule board I was pretending to read.

I picked up the cup. It was still hot.

"Thanks," I said quietly.

Grayson nodded.

We stood there in silence. The fluorescent lights flickered once. A gurney rolled past somewhere down the hall.

It was the most comforting moment I'd had in twenty-four hours.

And I had absolutely no idea what to do with that.
