Chapter 3

At eight the next morning, the fluorescent lights of the clinic stung my eyes. I sat in the vinyl chair and let the nurse fill a single, small vial. I had agreed to a compatibility screening, not a donation.

"Mrs. Mercer," the nurse said, tapping a pen against her clipboard. "When was the first day of your last cycle?"

I calculated the weeks in my head. The number made my pulse jump.

A rapid test led to a blood draw, which led to Dr. Thorne calling me into his private office.

"The lab work is conclusive," Dr. Thorne said, sliding a glossy printout across his metal desk. "You are nine weeks pregnant."

I picked up the ultrasound photo. A tiny, blurry shape rested in the center of the dark frame. I traced the edge of the glossy paper with my thumb. I did not think of Adrian in that second. I thought of Leo sitting on the living room rug three months ago, clutching his stuffed wolf.

"If I get a baby brother or sister, I will be the best big brother," Leo had promised, his chin jutting out with absolute certainty. "I will teach them how to build forts and how to hide from the monsters."

I slipped the photo into my purse. "Should I transfer your file to obstetrics?" Dr. Thorne asked, reaching for his keyboard.

I opened my mouth to tell him to wait, wanting Adrian to hear the heartbeat with me first.

A loud knock interrupted me. Another nurse hurried into the room and handed Dr. Thorne a thick yellow folder.

The doctor scanned the top page. His shoulders slumped.

"The matching report came back," Dr. Thorne told me, closing the folder. "You are the only screened donor who meets the safety threshold. But Mrs. Ward requires a large directed donation, not a standard pint."

"I just found out I am pregnant," I reminded him.

"I know," Dr. Thorne said, his tone turning clinical. "A large volume draw will cause severe hypotension. Your uterine perfusion will drop significantly. You risk losing the pregnancy entirely. I will inform your husband that you do not meet the donor criteria."

The heavy oak door swung open before I could respond. Adrian stepped inside, holding two paper cups of coffee.

"Adrian," I said, standing up from the vinyl chair. "I am nine weeks pregnant."

Adrian froze. The coffee splashed over the plastic lids, dripping onto his leather shoes. He turned his head and locked eyes with the doctor. "How dangerous would the donation actually be?"

The fragile warmth in my chest vanished, replaced by a cold, sharp dread.

"Extremely dangerous," Dr. Thorne answered sharply. "She faces severe blood loss. The fetus might not survive the drop in blood pressure."

Adrian set the leaking cups on the edge of the desk. "Can we draw less? Do it in batches? Just enough to get Sienna through the night?"

I stared at the man I married. "My body is not a corporate asset you can split and optimize, Adrian. This is a child."

Adrian rubbed his temples. "You have always been strong, Elena. You recover faster than anyone I know."

"Seven years ago, I stayed up all night drafting your hospital pitches," I said, my voice dropping to a harsh whisper. "You told me you would send me back to med school when the company stabilized. You promised me a future. Now you expect me to bleed out my pregnancy for your plus-one?"

"She is dying, Elena," Adrian shot back, his face flushing red. "This is not about your career or a gala. This is life and death."

"Then let the hospital treat her," I said. "My child is not collateral damage for your savior complex."

I pushed past him and walked straight to the intensive care wing. Adrian followed close behind.

Sienna lay in the center of a sterile white room, her skin the color of old parchment. An IV line ran into her bruised hand.

"Adrian, stop," Sienna whispered, turning her head as we approached the bed. "I do not want Elena to risk anything."

"You need the blood, Sienna," Adrian argued.

"Not at the cost of a baby," she said, her voice raspy.

She shifted her gaze toward the glass window. Nora sat on a plastic chair in the hallway, crying quietly into her knees.

"She already lost her father," Sienna murmured, tears spilling over her pale cheeks. "I do not know what happens to her if I die. She has no one else in this world."

Adrian looked at me, waiting for Nora's grief to change my answer.

"I am not doing this," I told Sienna, keeping my posture rigid. I refused to let them weaponize a little girl's grief against me. I turned to my husband. "And you will not discuss my medical decision with Leo. You leave our son out of this completely."

I walked out of the ward without looking back.

By three in the afternoon, the situation deteriorated. The monitors in Sienna's room shifted from steady beeps to a frantic, continuous alarm. Nurses rushed past me in the hallway carrying bags of saline.

Dr. Thorne intercepted me near the elevator bank. "Her oxygen and blood pressure are crashing. We have twelve hours."

The elevator doors chimed and slid open. Adrian stepped out.

He did not come alone.

Leo held Adrian's hand, his small face pale and terrified.

"Why is he here?" I demanded, marching forward to block my son's view of the ICU doors.

"The sitter canceled," Adrian lied, not breaking eye contact with me.

Leo peeked around my waist. He saw Sienna lying motionless through the glass. He saw Nora sobbing on the floor.

"Mom," Leo cried, grabbing the hem of my sweater. "You have to help her. She is our family now. She paints with me!"

I grabbed my son's arm and pulled him away from the glass. "Leo, go sit by the nurses' station."

"No!" Leo shouted, planting his sneakers firmly on the linoleum floor. "Dad said you are the only one who can fix it! Why are you letting her die?"

The accusation from my eight-year-old son felt like a physical blow. Adrian had dragged him into the one decision I had explicitly begged him to keep away from.

I looked at my husband. "You told him."

Adrian did not deny it. He only lifted a blue consent packet from the nurses' station and set it on the counter between us.

"He deserved to understand why everyone is scared," he said.

"He is eight." My voice shook. "He deserved to be protected from this."

Leo grabbed my sleeve with both hands. "Mommy, please. Dad said you're the only one who can save her."

Behind the ICU glass, another alarm began to sound. Dr. Thorne glanced at the monitor, then at me.

"Mrs. Ward is running out of time," he said quietly. "But my warning has not changed. A large-volume donation could end your pregnancy."

Leo's fingers tightened around my sleeve. Adrian pushed the consent packet one inch closer.

For the first time, the choice had a piece of paper waiting for my signature.
