Chapter 4

Bria's whiteboard filled up fast. By day two, it wasn't just my name at the top anymore. There were arrows now, names branching off into little boxes—hospitals, dates, a column marked "AMA" that she'd circled twice.

"AMA," she said, tapping the marker against it. "Against medical advice. That's the phrase you use when a patient bails on treatment before anyone can prove what's actually wrong with them." She looked at me over her shoulder. "You said she did that to you. Turns out she's done it before. To other doctors. In other states."

I set my coffee down slowly. "How do you know that?"

"I called it research for a profile piece. Cardiac patient advocacy, blah blah, very sympathetic, very boring." She shrugged, but her eyes were sharp. "Two hospitals. Different cities. Both times she checked herself out right after some poor cardiologist put a lot of time and attention into her. Inconclusive workups. No diagnosis ever actually confirmed."

I stared at the board. At the pattern taking shape in blue marker, a shape I recognized the way I recognized an arrhythmia on a strip—not proof yet, but a rhythm that wasn't right.

"She's done this before," I said quietly. "Before me."

"Looks that way."

I pressed my thumb into my wrist and didn't say anything else. Outside, the wind rattled the old window frame, and somewhere down on the street a car alarm went off and stopped.

The knock came two nights later, hard enough to rattle the chain lock.

Bria got there first. She opened the door only as far as the chain allowed, one eyebrow already climbing.

"Atlas." She said his name like she was identifying a stain.

I came around the corner from the kitchen and stopped in the hallway, out of sight, close enough to hear.

"I need to speak to Margot." His voice was wrong. Too fast, too raw. Not the boardroom voice. "It's an emergency."

"She's busy."

"Bria—"

"She'll be with you in a moment." Bria's voice didn't rise. It didn't need to. She shut the door in his face and turned to look at me, and something in her expression asked a question I answered by walking forward.

I opened the door myself.

He looked terrible. His coat was unbuttoned in the cold, his hair pushed back like he'd been dragging his hands through it for an hour. There was a gray cast under his eyes I'd never seen on him, not once, not in ten years.

"Valentina's in acute heart failure." He said it fast, like the words might not survive if he slowed down. "She's crashing. The team at Presbyterian says she needs surgery tonight, and they say—" His jaw worked. "They say you're the only one who knows her history well enough to go in safely. I need you to operate."

I looked at him. I let the silence stretch long enough that I watched him understand, in real time, what he'd just asked of the woman he'd had escorted out of a lobby with security's hand on her elbow.

"You froze my accounts," I said. "You revoked my privileges. You let them walk me out the front door of a hospital I built six years of my life inside, and you didn't lift your eyes off your phone to watch it happen."

"Margot, she doesn't have time—"

"I know exactly how much time she has." My voice stayed even. That was the part that seemed to frighten him most—that I wasn't shouting. "I'll operate."

Something in his shoulders dropped, relief moving through him so fast it looked like pain.

"But first," I said, "you'll sign over ten percent of Hoffman Group equity into my name. Documented. Legally binding. Tonight, before I set foot in that OR."

He stared at me like I'd spoken in a language he didn't recognize.

"That's—" His voice cracked, then hardened into something closer to fury, the fury of a man discovering the floor had moved without his permission. "That's extortion, Margot. You can't hold a woman's life hostage over equity. It's unconscionable."

"Unconscionable." I let the word sit in the cold air between us. "You froze every account with my name on it in the time it took me to walk from the ICU to the front doors. You revoked my medical license before an investigation even started. You want to talk to me about what's unconscionable."

"There's no time for this." He was pacing now, one hand raking through his hair again. "She could die, Margot. Do you understand that? She could actually die."

"Then call your lawyer." I crossed my arms and leaned against the doorframe, and I did not move. "I'm sure he can draft ten percent faster than you think."

He looked at me for a long moment—really looked, the way he hadn't in the lobby, the way he hadn't in months. Something in his face was breaking apart and rebuilding itself at the same time, some old certainty about who I was giving way under the weight of who I'd actually become.

The clock on Bria's wall ticked. Somewhere down the hall a door closed. He pulled out his phone.

His hands were shaking when he dialed. I watched him give instructions in a voice stripped of all its usual command, watched him repeat the terms twice to make sure the lawyer on the other end understood he was not joking, watched him hang up and stand there in the doorway of an apartment that wasn't mine, in a life that no longer had room for the version of me he thought he'd left behind.

"It'll be signed within the hour," he said.

"Good." I reached for my coat off the hook behind Bria's door. "Then let's go save her life."
