Chapter 2

The ICU door swung open, and Kale stepped in first, his white coat crisp, shoulders squared, expression sharp as steel. He glanced at me once, cold, then softened completely as his eyes landed on Silvia.

“You’re awake… I handled the reporters. Don’t worry. They swarmed you, but you’re okay. We got through it.”

Silvia clutched her bouquet tighter, tears running down her cheeks. “I… I’m sorry. Marion, it’s my fault. I shouldn’t have had the surgery. I… I can give it back to you—”

“No,” Kale said firmly, turning to her with that soft smile reserved only for her. He brushed a strand of hair from her face. “The surgery went perfectly. You’re recovering, Silvia... Don’t even think about that.”

Then his gaze snapped back to me,“And you… You caused a scene in front of everyone. Embarrass me as your husband! Put Silvia in a terrible spot. Control yourself, will you?”

My mom, standing just behind Kale, added calmly, “Marion… Silvia is your sister now. You need to accept that. She’s part of this family.”

“Accept that?” I whispered, “I’ve spent my whole life being left behind! Ignored! Broken! And now you hand over my heart—my chance to live—to her?”

“Marion, enough!” Kale’s jaw tightened. “Don’t speak like that to her. Silvia is fragile right now. She’s recovering. Show some restraint.”

“My restraint?” I laughed, bitter while tears streaking my face. “I’ve been polite, careful, considerate for twenty-five years! I’ve stepped aside every time, ignored my pain, because I understood you were busy.”

Kale didn’t move. His tone was sharp. “And now? Now you act like a child. You’re endangering yourself with this attitude. Control it, Marion. For your own good.”

I turned to Silvia, voice shaking. “Do you know what they did for you? VIP room, top specialists, the donor heart I waited three years for… and for me? Left me in a normal ward, left me alone with nurses, told me to wait, wait, fucking wait!”

Silvia shook her head, and cried harder. “I… I didn’t know… I can wait, Marion… I can—”

Kale’s hand went to her shoulder again, firm but tender. “Silvia, don’t. You are fine. This isn’t your fault.”

I glared at him. “Wait for what? Your heart’s nearly normal. Are you waiting for me to die so you can feel okay about yourself?”

Kale’s eyes snapped to mine, hard as stone. “Don’t speak to me like that! I'm your husband here!” His voice echoed with authority. “You’re alive because of care. Respect it.”

My mom spoke up again, calm, decisive. “Marion! You need to be understanding, just like before. We are doctors here and we knew your heart more than anything else. Silvia needs it first! Be more understanding!”

I shook my head, chest burning. “Understanding? I’ve been understanding all my life. And you! Both of you chose her over me. My life, my chance, my heart was handed to her, and you smile about it. Are you all proud of that?”

The hospital room went completely silent. Even the machines seemed quiet. I could feel the blood drain from my mom’s face. Her lips trembled, like she didn’t know how to speak.

“You… you…” she stammered, pointing at me, finger shaking.

I leaned back against the headboard, numb. “What about me?” I croaked. “Didn't you always say a doctor’s family has to hold themselves to a higher standard? That we put others first? So fine—donate. Give your heart to Silvia. I’ll wait for my own. That way nobody owes anyone. Isn’t that perfect?”

Kale’s jaw tightened. He stepped closer, voice low, hard. “Marionette… enough! Don’t talk like that. Not here.” But then he glanced at Silvia, and all the hardness melted away. His hand rested on her shoulder, gentle, protective.

My dad’s face went red. “How can you speak to your mother like that?”

Silvia dropped to her knees with a thud, sobbing so hard she could barely breathe. “It’s my fault! I’ll go find the doctor right now! I’ll give the heart back to Marion! Dr. Ashford saved me and I don’t even deserve to live!”

She scrambled toward the door, but my mom swooped in and grabbed her, hugging her tight. “Silvia, stop. I won’t let you do that.”

Then my mom turned to me. Her eyes were sharp, cold, full of disappointment. “Marionette… I never thought you’d become like this. Silvia just went through major surgery, and you’re telling her to remove the heart? You’re trying to kill her!”

“I just… I want to live too,” I whispered, every word shaking. “Is that so wrong?”

“Marionette… listen carefully. Silvia is my daughter now. I’ll take care of her until she fully recovers. You… when you can be calm, when you can apologize, then we’ll talk.”

She grabbed Silvia’s hand and walked out without even glancing at me. My dad and Kale hesitated and looked at me, but then followed them.

Through the ICU window, I watched them in the hallway. My mom gently wiped Silvia’s tears. Kale’s hand rested lightly on her back, protective, gentle. My dad leaned down to comfort her. They all merged together under the lights like they were one perfect little unit and I wasn’t in it.

I lay back on the bed, listening to my own heartbeat. Slow. Weak. Broken. And I realized something worse than dying… was being abandoned by everyone you trusted most.

Dr. Antonio hesitated for a long time before speaking. He glanced at the door, then leaned closer to my bed and lowered his voice.

“Actually… a donor heart came in last week. It was a match for you. A very good match. But Dr. Kale Ashford and your parents signed the paperwork to redirect it to another patient.”

I stared at him. I thought I misheard.

“There must be a mistake,” I said quietly. My throat burned with every word. “You’re saying there was another heart… and she gave it away?”

Dr. Antonio looked uncomfortable. “Your case was more critical. Everyone in the department knows that. The other patient needed a transplant too, yes, but you were deteriorating faster. We didn’t think it was right.”

Of course they didn’t.

Kale and my parents had to prove they're fair. They had to prove they weren't giving special treatment to me. They would rather let me die than let people think they're biased.

“Thank you for telling me, Dr.” I said through the oxygen mask.

“Marionette, don’t give up,” Dr. Antonio urged. “I’ve contacted other transplant centers. I’ll keep pushing, okay? You still have options.”

“No,” I said. “Please don’t.”

He froze. “What?”
