The instant Clara’s body hit the sand, my world shattered.

Only moments before, she had been sprinting toward me, her laughter carried by the sea breeze as the waves chased her tiny feet along the shore behind our villa. Then, in a blink, the light in her eyes dimmed—her knees buckled, and she folded into the earth like a marionette whose strings had been mercilessly severed.

“Clara!” My scream tore through the quiet air as I rushed forward—but Leonardo reached her first.

One minute, he’d been standing near the fireplace, talking business with his men. The next, he was kneeling beside her, scooping her up into his arms with a desperation that stripped away every trace of the feared man people called the mafia king.

“Clara, piccola,” his voice cracked, trembling with panic. “Stay with me, baby.”

Her tiny hands clawed at his shirt, her breaths coming in ragged gasps. “Papa…”

“I’m here, sweetheart.” His voice broke as his eyes darted to mine. “Call the doctor. Now.”

My hands shook as I fumbled for my phone. “She—she just collapsed,” I stammered when Dr. Prietto answered. “She can’t breathe.”

“Bring her here immediately,” he ordered.

Just moments ago she’d been fine—laughing, glowing with life. This couldn’t be real.

“She was fine, Leonardo,” I whispered weakly, my heart pounding.

“Not now,” he snapped, his jaw set as he lifted Clara and strode to the waiting SUV. His men moved quickly, opening the doors.

I climbed in after them, tears streaming down my face. “Hold on, baby. Mommy’s here.”

Leonardo said nothing. His body was rigid, his jaw tight, but I could see it—the fear beneath that steel composure.

The drive to the hospital felt endless, every second a lifetime.

When we arrived, doctors were already waiting. Leonardo didn’t pause; he stormed through the entrance, barking orders with the authority of a man used to control—but this time, his voice was laced with fear.

Dr. Prietto met us halfway, his expression grim as the nurses took Clara.

“What’s happening to my daughter?” Leonardo demanded, his voice cold and sharp.

The doctor exhaled. “It’s what we were afraid of—her nervous system is deteriorating rapidly. We must act fast—”

“Then do it,” Leonardo cut in, his tone deadly calm.

Dr. Prietto hesitated. “She needs a transfusion. But her blood type is extremely rare. Only one person matches.”

I froze as his eyes met Leonardo’s.

“You.”

Leonardo’s expression didn’t waver, but I saw the muscle in his jaw twitch.

“Do it,” I said quickly. “Whatever it takes.”

Before the doctor could respond, the sound of heels echoed through the hall.

A woman appeared—tall, poised, wrapped in an expensive coat. Her face was pale, her eyes frantic.

Ariana.

Leonardo’s first love—the woman who had vanished years ago, leaving him shattered and hollow. I was the one who’d helped him heal, the one he eventually married.

But she wasn’t alone.

A small boy clung to her hand. He looked frail—about Clara’s age, with the same ghostly pallor that turned my blood cold.

No. Please, not now.

Dr. Prietto stiffened. “This complicates things.”

Leonardo’s voice was clipped. “Why is she here?”

Ariana’s lip trembled. “Because Dr. Prietto is my son’s physician. Cristian needs help too.”

The air thickened.

Dr. Prietto sighed heavily. “Cristian suffers from the same condition as Clara. And Leonardo… you’re his only donor match.”

The words hit like a gunshot.

“What?” Leonardo’s voice turned deadly quiet.

Dr. Prietto hesitated before saying, “Cristian is your son.”

The room went still.

My breath caught. The world tilted and blurred around me.

Leonardo’s hands balled into fists. “This isn’t funny.”

Ariana’s eyes filled with tears. “I never wanted you to find out like this.”

“How long?” His tone was sharp, dangerous.

“Eight years.”

I flinched. Eight. Clara’s age.

Leonardo’s nostrils flared. “And you never told me?”

Tears rolled down her cheeks. “I was protecting him.”

He let out a bitter laugh. “From me?”

“From my late husband,” she whispered. “If he knew the truth, he would’ve killed us both.”

My stomach turned. None of this felt real.

Dr. Prietto’s voice broke through the tension. “There’s no time. Leonardo, both of your children need you.”

Both. The word echoed painfully in my head.

Leonardo stood frozen, his expression unreadable.

Ariana’s voice shook. “Please… save my son.”

My voice barely came out. “Leonardo… save our daughter.”

His eyes moved between Clara’s pale face and Cristian’s trembling frame.

One decision. Two lives. No right answer.

And in that moment, I understood something terrible—

Even the strongest man can bleed.
