Chapter 4

The fluorescent lights of the ER hit my eyes like needles. Voices swirled around me—familiar voices, colleagues speaking in the clipped, urgent shorthand of medical emergencies. I caught fragments: 'BP's dropping,' 'X-ray stat,' 'Get Dr. Howard on the phone.' Someone cut away my scrubs. Someone else started an IV. I heard Dr. Chen's voice, steady and professional, giving orders. I wanted to tell him I was fine, that this was just a temporary inconvenience, but the words wouldn't come. My hands lay on the gurney like broken birds, and I couldn't bring myself to look at them.

The imaging suite was a blur of mechanical whirrs and cold metal surfaces. X-rays. CT scans. The technicians spoke to each other in hushed tones, occasionally glancing at me with the particular look medical professionals reserve for colleagues who've crossed the line from healer to patient. I knew that look. I'd worn it myself, standing beside beds just like this one.

Dr. Marcus Reynolds, the orthopedic surgeon on call, stood at the light board, scrolling through the scans. His face was grave in the blue glow of the images. I watched his eyes move across the screen—the same eyes that had shared a joke with me in the elevator just last week. Now they were seeing something different. Something irreparable.

'Multiple comminuted fractures across both hands,' he said finally, his voice low and measured. 'The metacarpophalangeal joints are essentially crushed. Tendon damage, nerve damage... Sara, I need to be honest with you. This is... significant.'

He turned to face me, and I saw something I'd never seen before in a doctor's eyes when looking at me: pity.

'What does that mean?' I asked. My voice sounded foreign to my own ears.

'It means we're going to do everything we can,' he said. 'But you need to prepare yourself. Surgery can fix a lot, but...'

He didn't finish the sentence. He didn't need to.

When I woke, I was in the ICU. The first thing I noticed was the silence. No beeping monitors, no voices, no sound at all. Then I understood: the silence was in me. The constant, subliminal hum of my own thoughts—the mental catalog of tasks, patients, schedules—had gone quiet. In its place was something vast and empty.

I looked down at my hands.

They were encased in metal frames—external fixators, I recognized dimly. Surgical pins pierced through skin and bone, holding everything in alignment. Both hands, immobilized completely. I stared at them for a long time, trying to process what they were telling me.

'Dr. Spencer?'

Chloe Park sat in the chair beside my bed. Her eyes were red-rimmed, her cheeks wet. She held my forearm—the only part of me that wasn't broken—and she was crying silently, her shoulders shaking with the effort to stay quiet.

'Hey,' I whispered.

She looked up, startled, then quickly wiped her face. 'You're awake. I'll get the nurse.'

'Chloe.' I kept my voice steady. 'What are they saying?'

She hesitated, then reached for my chart with her free hand. 'The surgeries went well, but... there's a lot of damage. Dr. Reynolds said the next few months will determine... everything.'

She couldn't say it. Couldn't say what we both knew: my hands might never work the same way again. I might never operate again.

I looked at the ceiling instead. The tiles were acoustic, dotted with tiny holes. I counted them. One, two, three...

So this is what it costs.

Two days later, Dalton appeared. He carried a bouquet so large it looked like a funeral arrangement, all white lilies and baby's breath. His suit was perfectly pressed, his hair immaculate. He looked like he'd stepped out of a magazine spread titled 'Concerned Husband Visits Injured Wife.'

He adjusted his cufflinks before sitting down. A habit. A tell.

'Sara, God, I came as soon as I could.' His voice was pitched to carry emotion. 'The phone died that night. I had no idea until morning.'

I watched him. Just watched him.

'I must have missed your calls. I didn't see them until—'

'Your phone didn't die, Dalton.' My voice was quiet. Very quiet. 'You were with her.'

He paused, recalibrating. 'What are you talking about?'

'Jazmine Rivera. Your protégée. Your mistress. You were with her that night.'

'Sara, you're not thinking clearly. You're on medication, you're—'

'I want a divorce.'

He laughed. Short, incredulous. 'You don't mean that. You're upset, you're hurt, but—'

'I want a divorce.'

He stopped laughing.

Three days later, Dr. Richard Hale walked into my room. Dalton introduced him as 'the best hand specialist on the East Coast.' Dr. Hale was tall, silver-haired, with the confident bearing of someone who charged by the minute and billed by the hour.

'Your husband has arranged for me to take your case personally,' Dr. Hale said, his tone making it clear this was a favor being done. 'We'll need multiple surgeries, extensive rehab, but with the right care, there's every reason to be optimistic.'

Dalton beamed. He'd solved the problem. He'd fixed me.

I looked at Dr. Hale's perfectly manicured hands as he examined my ruined ones, and I understood the transaction perfectly. Fix my hands, buy my compliance, restore the status quo. I would be his good, grateful wife again. His accomplished surgeon wife, successful but never too successful. Never threatening.

'When can we start?' I asked.

Dr. Hale smiled. 'We'll schedule the first procedure as soon as you're stable.'

As soon as he left, I reached for my phone with my elbow. I had contacts. Colleagues. Friends. People who owed me favors. I began typing with my nose, slow and painstaking, but determined.

This was my life. These were my hands. And I would cut Dalton out of their recovery if it was the last thing I did.
