Chapter 5

Dr. Marcus Webb caught me in the corridor outside the surgical wing.

I had just finished a valve repair. My scrubs were still damp at the collar. I was thinking about the patient's daughter, who had waited six hours in the family room and cried when I told her everything went well.

Webb stepped into my path.

"Isabelle," he said. "A moment?"

I stopped.

Webb was fifty-three. Silver hair. Steady hands. He had trained at Stanford and published more papers than anyone else in the department. He was the kind of surgeon who never raised his voice because he never needed to.

He held a tablet.

"I reviewed a cardiac advisory that came through the system," he said. "Patient named Theodore Richards. Monarch Syndrome. Significant progression."

I nodded. I waited.

"The advisory was signed by you," he said. "Dr. I. Dixon."

My hands were very still at my sides.

"I'm not the treating physician," I said. "I reviewed the monitoring data and issued the advisory through the corporate health plan. Standard protocol."

Webb looked at me. His eyes were calm.

"Are you personally connected to this patient?" he asked.

The question was careful. Professional.

"No," I said.

It wasn't a lie. Not anymore.

Webb nodded slowly.

"Whoever monitors this patient should know something," he said. "The cardiac timeline is accelerating faster than the standard Monarch progression model predicts. The intervals on his last EKG are compressed. If he experiences acute stress — physical or emotional — the arrhythmia could cascade into full arrest within minutes."

He handed me the tablet.

"Minutes," he repeated. "Not hours."

I took the tablet. I looked at the screen.

The butterfly pattern was there. But the wings were folding in on themselves.

"Thank you," I said.

Webb studied my face.

"Isabelle," he said quietly. "If you need to recuse yourself from anything related to this case —"

"I don't," I said.

He nodded.

He walked away.

I stood there in the corridor. The tablet was cool in my hands.

I returned it to the nurses' station.

Then I went to the locker room. I changed into street clothes. I drove home.

I did not call Theo.

I had already sent the advisory. I had already done my job.

The rest was his choice.

Joshua Tree at night is cold.

Theo stood outside the glamping tent Dior had booked — canvas walls, king bed, heated floors, nothing like actual camping — and looked up at the stars.

His hands were shaking.

Just a little. Just enough that he noticed when he tried to buckle his climbing harness.

He told himself it was the cold.

The desert air dropped thirty degrees after sunset. Everyone's hands shook in cold like this.

He buckled the harness on the second try.

Dior came out of the tent. She was wearing climbing gear that looked like it had been styled by a magazine editor. Everything matched. Everything fit.

"Babe," she said. "The photographer wants to do a night shoot. You and me. Stars in the background. It'll be gorgeous."

Theo looked at her.

"I'm tired," he said.

She laughed. "You're always tired. Come on. Five minutes."

She pulled him toward the rock face. The photographer was already setting up lights.

Theo stood where Dior told him to stand. He smiled when she told him to smile. He put his arm around her when she leaned into him.

The camera flashed.

His hand trembled against her waist.

She didn't notice.

The photographer didn't notice.

Theo told himself it was nothing.

Just cold.

Just nerves.

Just the altitude.

They went back to the tent. Dior fell asleep with her phone in her hand, scrolling through the photos, choosing which ones to post.

Theo lay awake.

His chest felt tight. Not painful. Just — tight.

He thought about the advisory. The one that had come through his email last week.

He hadn't read it carefully. Just skimmed it. Something about avoiding strenuous activity.

But Dior had already booked everything. The photographer. The permits. The whole production.

And he was fine.

He was always fine.

He closed his eyes.

He did not sleep well.

***

Morning came fast.

The sun turned the rocks orange. The air was still cold. Dior was already awake, already dressed, already talking to the photographer about angles.

Theo buckled his harness again.

His hands were steadier now. The tremor was gone.

See? Just cold.

They started the climb at seven.

The route was rated 5.8. Moderate difficulty. Dior went first. She was surprisingly good — her family had paid for lessons at some indoor gym in Malibu, and she moved up the rock with the confidence of someone who had been told she was excellent at everything.

Theo followed.

The photographer stayed below, camera trained upward.

Theo's hands found holds. His feet found ledges. He climbed.

Fifty feet.

Seventy-five feet.

One hundred feet.

His chest was tight again.

He ignored it.

One hundred twenty-five feet.

Dior was thirty feet above him now, anchored to a ledge, shouting encouragement that echoed off the granite.

"You've got this, babe! You're amazing!"

Theo reached for the next hold.

His hand closed around the rock.

And then his heart stopped.

Not metaphorically.

Literally.

The Monarch Syndrome triggered. The arrhythmia cascaded through his chest like a wire snapping under too much current. The butterfly pattern collapsed into itself.

His vision went white.

His hands lost their grip.

He fell backward — two feet, three feet — before the harness caught him.

He hung there. Limp. His face ashen. His chest not moving.

Below, the photographer dropped his camera.

Above, Dior started screaming.

Not instructions.

Not his name.

Just screaming.

Raw. Unfiltered. The sound of a woman who had never once faced a consequence she couldn't perform her way out of.

And now she was thirty feet above a man who was dying.

And there was no script.

No photographer.

No audience that could save him.

Just the desert.

And the rock.

And the silence after her screaming stopped.
